Death Certificates

To Request a Certified Copy of a Death Record:

Certified copies of death certificates can be issued for anyone who died in New Hartford, or who resided in New Hartford at the time death.  If the person lived in and died in another town, please contact the Town Clerk of the town where the death occurred.

Any person 18 years of age or older may purchase certified copies of death records which are at least 100 years old.  The cost is $20.00 per copy payable by cash or check - credit cards are not accepted.

For anyone who died on or after July 1, 1997, certified copies of death certificates that show the social security number of the deceased will only be issued to the licensed funeral director or embalmer who completed the certificate, the surviving spouse or next of kin.  Certified copies of death certificates with the social security number redacted will be issued to anyone else.

Any eligible party must submit the following documentation:

1. The Request for Copy of Death Certificate form.
2. A valid government issued photographic identification; or if a photo id is not available, at least two of the following documents:
   • a social security card
   • written verification of identity from employer
   • automobile registration
   • copy of utility bill showing name and address
   • checking account deposit slip showing name and address
   • voter registration card
3. Cash or Check (payable to Town of New Hartford) for $20.00 per certificate

Copies may be requested in person or by mail.
